Jevtana is an anticancer medicine that contains the active substance Cabazitaxel. It belongs to a class of medications called “taxanes” (antimicrotubule agents). It is supplied as a kit containing a vial of concentrate (60 mg/1.5 ml) and a vial of solvent for dilution. The solution is clear to slightly cloudy, colorless to pale yellow.

2. Mechanism of Action:
Cabazitaxel works by disrupting the growth and division of cancer cells. It binds to a protein inside cells called tubulin, which is essential for cell division. By stabilizing tubulin structures, it prevents cancer cells from dividing and multiplying, ultimately leading to their death.

3. Therapeutic Indications (Uses):
Jevtana is used in combination with a corticosteroid medicine called prednisone or prednisolone for the treatment of adults with:

  • Prostate Cancer: It is indicated for the treatment of patients with metastatic castration-resistant prostate cancer (mCRPC) who have previously received a chemotherapy regimen containing docetaxel. In other words, it is used for advanced prostate cancer that no longer responds to hormone therapy and has progressed after treatment with docetaxel.

4. Warnings and Precautions:
Jevtana can cause serious side effects. Close monitoring by your healthcare team is essential.

  • Bone Marrow Suppression (Neutropenia): This is a common and potentially severe side effect. It means a significant decrease in white blood cells (especially neutrophils), increasing the risk of serious and potentially fatal infections. Frequent blood count monitoring is required.

  • Allergic Reactions (Hypersensitivity): Severe allergic reactions can occur during the infusion. Symptoms may include rash, itching, difficulty breathing, flushing, dizziness, or chest pain. Premedication with antihistamines and corticosteroids is usually given before each dose.

  • Gastrointestinal Problems:

    • Diarrhea: Severe diarrhea is common and can lead to dehydration. Immediate treatment and fluid intake are necessary.

    • Nausea and Vomiting: Anti-emetic medications are often prescribed to manage these symptoms.

    • Gastrointestinal Bleeding and Perforation: Risk of bleeding or holes in the stomach or intestines, which can be fatal.

  • Kidney Problems (Renal Failure): Cabazitaxel can cause kidney failure, sometimes fatal. Adequate hydration is important.

  • Liver Problems: Patients with mild liver impairment may be at increased risk of severe neutropenia. It is not recommended for patients with severe liver impairment.

  • Lung Problems (Interstitial Lung Disease/Pneumonitis): Inflammation of the lungs can occur and be fatal. Report any new or worsening respiratory symptoms.

  • Hematuria (Blood in Urine): Bleeding in the bladder, including cystitis, has been reported.

  • Peripheral Neuropathy: Nerve damage causing numbness, tingling, or burning pain in the hands and feet.

  • Pregnancy and Contraception: Jevtana can cause fetal harm. Men with pregnant partners should use effective contraception during treatment and for 3 months after the last dose. It is not known if it passes into breast milk.

5. Administration:
Jevtana must be prepared and administered by a healthcare professional. The concentrate must first be mixed with the provided solvent, and then this mixture is further diluted in an infusion bag. It is given as a one-hour intravenous (IV) infusion every three weeks. Prednisone or prednisolone is taken orally daily throughout treatment.

6. Storage:
Store the unopened vials at room temperature (below 25°C). Do not refrigerate. Store in the original package to protect from light. The diluted solution should be used immediately, but if necessary, it can be stored for short periods under refrigeration.
