Yondelis (active substance: trabectedin) is an anticancer agent used to treat certain types of solid tumors. The drug binds to DNA within cancer cells, disrupting DNA repair mechanisms and causing cell cycle arrest and tumor cell death. Yondelis is administered by intravenous infusion under the supervision of an oncology specialist.

Indications:

  • Treatment of advanced or metastatic soft tissue sarcoma (depending on histologic subtype and clinical assessment).
  • Use in soft tissue sarcomas after failure of standard therapies or when other options are inappropriate.
  • May be used in combination treatment protocols or clinical trials for other tumor types as recommended by an oncologist.

Dosage and administration:

  • Administered intravenously via central or peripheral line after appropriate dilution and preparation per manufacturer and institutional protocols.
  • Dose and scheduling are determined by the treating physician based on tumor type, patient condition, and liver/kidney/bone marrow function.
  • Do not change dose or discontinue treatment without medical advice.

Warnings and precautions:

  • Liver effects: Yondelis can cause elevations in liver enzymes and other hepatic adverse effects. Liver function tests (ALT, AST, bilirubin) should be checked before each cycle and during therapy. Dose delay or modification may be required for elevated liver tests.
  • Bone marrow suppression: Yondelis may cause decreases in blood cell counts (neutropenia, anemia, thrombocytopenia). Complete blood counts should be monitored regularly and supportive measures used as needed (e.g., transfusions, growth factors).
  • Infection and bleeding: Due to bone marrow suppression, infection and bleeding risks are increased; report fever, signs of infection, or bleeding immediately.
  • Pregnancy and breastfeeding: Trabectedin may cause fetal harm. Avoid use during pregnancy. Women of childbearing potential should use effective contraception during treatment and for a recommended period afterward. Breastfeeding is not advised during therapy.
  • Hypersensitivity and infusion reactions: Allergic reactions and infusion-related symptoms may occur. Monitor for rash, respiratory symptoms, or severe hypersensitivity and stop infusion if severe reaction develops.
  • Renal function: Assess renal function before starting therapy and monitor as clinically indicated.
  • Drug interactions: Inform the physician of all concomitant medications and supplements. Drugs affecting hepatic enzymes may alter trabectedin exposure and toxicity.
  • Patients with poor performance status or comorbidities: Dose adjustments or withholding therapy may be required based on overall condition and comorbid diseases.

Common side effects:

  • Nausea, vomiting, and loss of appetite.
  • Fatigue and asthenia.
  • Decreases in blood cell counts (neutropenia, thrombocytopenia, anemia).
  • Elevated liver function tests.
  • Muscle or joint pain.
  • Constipation or diarrhea.

Important notes:

  • Yondelis is administered by intravenous infusion only in a medical facility equipped for oncology care.
  • It is available by prescription only and should be used under the supervision of an oncologist.
  • Adherence to scheduled monitoring (blood counts, liver and kidney function tests) is essential to minimize risks.
  • Do not share medication; dispose of materials according to medical waste protocols.
