Xgeva is a medication containing the active ingredient Denosumab, a monoclonal antibody that works by inhibiting the RANKL protein, which is responsible for activating cells that break down bone tissue.
The medication helps reduce bone loss and prevent skeletal-related complications associated with certain types of cancer. It is also used to treat specific bone tumors and elevated calcium levels in the blood.
Indications
-
Prevention of skeletal-related events, such as pathological fractures and spinal cord compression, in patients with multiple myeloma.
-
Prevention of skeletal-related complications in patients with solid tumors that have spread to the bones.
-
Treatment of Giant Cell Tumor of Bone in adults and skeletally mature adolescents when surgery is not possible or may cause severe complications.
-
Treatment of hypercalcemia of malignancy in patients who have not responded to bisphosphonate therapy.
Dosage and Administration
-
Usual dose for patients with bone metastases and multiple myeloma: 120 mg once every 4 weeks.
-
For Giant Cell Tumor of Bone: 120 mg every 4 weeks, with additional doses on days 8 and 15 of the first month.
-
For hypercalcemia of malignancy: 120 mg every 4 weeks, with additional doses on days 8 and 15 of the first month.
-
Administered by subcutaneous injection into the upper arm, thigh, or abdomen by a healthcare professional.
-
The physician may prescribe calcium and vitamin D supplements to prevent low blood calcium levels, unless calcium levels are elevated.
Warnings and Precautions
-
May cause severe hypocalcemia (low blood calcium levels), and any existing calcium deficiency must be corrected before starting treatment.
-
Blood calcium levels should be monitored regularly, especially in patients with severe kidney impairment.
-
May cause osteonecrosis of the jaw. A dental examination is recommended before treatment, and invasive dental procedures should be avoided whenever possible during therapy.
-
May increase the risk of atypical femoral fractures.
-
Multiple vertebral fractures may occur after discontinuation of treatment. Therefore, treatment should not be stopped without consulting a physician.
-
Elevated calcium levels may occur after discontinuation, particularly in patients with Giant Cell Tumor of Bone.
-
Contraindicated in patients with uncorrected hypocalcemia or known severe hypersensitivity to Denosumab.
-
May harm an unborn baby. Pregnancy should be avoided during treatment and for at least 5 months after the last dose.
-
Should not be used simultaneously with any other medication containing Denosumab.
Possible Side Effects
-
Fatigue and general weakness.
-
Nausea.
-
Diarrhea or constipation.
-
Back, joint, and limb pain.

Headache.
-
Low calcium and phosphate levels in the blood.
-
Anemia.
-
Swelling of the extremities.
-
Decreased appetite.
-
Skin rash.
-
Shortness of breath.
-
Osteonecrosis of the jaw or atypical femoral fractures, which are serious complications requiring medical evaluation.
-
Severe allergic reactions in rare cases.
Note: Xgeva is a prescription medication that must be used under the supervision of a qualified healthcare professional. Calcium levels and dental health should be monitored throughout treatment. The medication should not be discontinued, and dosing schedules should not be adjusted without consulting a physician.
