Polivy is an antibody–drug conjugate used in oncology. It targets the CD79b antigen expressed on B-cell malignancies to deliver a cytotoxic payload into cancer cells, causing cell death. Polivy is administered by intravenous infusion under the supervision of an oncology specialist and is used mainly for certain B-cell non-Hodgkin lymphomas.
Indications:
- Treatment of diffuse large B-cell lymphoma (DLBCL) in adult patients who have relapsed or are refractory to prior therapies.
- Typically used in combination regimens with other chemotherapy agents as determined by the treating physician.
- May be used in specific settings related to stem cell transplantation or clinical trials as recommended by an oncologist.
Dosage and administration:
- The 140 mg vial is diluted and administered as an intravenous infusion per manufacturer and institutional protocols. Dose, schedule, and infusion rate are determined by the treating physician based on patient weight, disease status, and concomitant treatments.
- Administer in a healthcare setting equipped to manage oncology infusions and potential infusion reactions.
- Do not change dose or stop therapy without medical advice.
Warnings and precautions:
- Bone marrow suppression: Polivy can cause cytopenias (neutropenia, thrombocytopenia, anemia). Monitor complete blood counts before each cycle and during treatment; provide supportive care (growth factors, transfusions) as needed.
- Infections: Increased risk of infections, including serious and potentially life-threatening infections. Monitor and treat promptly.
- Infusion-related and hypersensitivity reactions: Monitor patients during and after infusion. Stop or slow the infusion and treat if severe reactions occur.
- Peripheral neuropathy: May cause sensory or motor peripheral neuropathy (numbness, tingling, pain, or weakness). Evaluate symptoms and consider dose modification or discontinuation if neuropathy worsens.
- Hepatic and renal function: Assess liver and kidney function before starting treatment and periodically thereafter. Dose adjustments may be necessary for organ impairment.
- Pregnancy and breastfeeding: May cause fetal harm. Avoid during pregnancy; use effective contraception during treatment and for a recommended interval afterward. Do not breastfeed during therapy.
- Drug interactions: Inform the treating physician of all medications and supplements.
- Patients with poor performance status or comorbid conditions may require dose adjustments or more intensive monitoring.
Common side effects:
- Cytopenias (neutropenia, anemia, thrombocytopenia).
- Nausea, vomiting, decreased appetite.
- Fatigue.
- Infusion reactions (fever, chills, rash).
- Peripheral neuropathy symptoms (numbness, paresthesia).
- Elevated liver enzymes.
- Diarrhea or constipation.
Important notes:
- Polivy is for intravenous administration only and must be given in an appropriate medical facility.
- Available by prescription only and should be used under the supervision of an oncologist or hematologist.
- Regular monitoring (CBC, liver and kidney function tests) is essential.
- Seek immediate medical attention for severe allergic reactions or signs of serious infection.
- Do not share medication; dispose of medical waste properly.
